# Managing Chronic Pain with CBT: Breaking the Boom-and-Bust Cycle

When managing long-term musculoskeletal pain, chronic fatigue or recurring health flare-ups, the physical symptoms are entirely real. Appropriate medical care is always the first priority. Yet the daily reality of unpredictable symptoms often creates a secondary layer of difficulty, including frustration, fear of movement and a punishing cycle of overexertion followed by exhaustion.

Cognitive behavioural therapy does not cure underlying physical pathology. Instead, it provides practical strategies to manage persistent pain, helping individuals break unhelpful behavioural cycles, calm nervous system hyper-arousal and regain a more predictable quality of life.

## Understanding the boom-and-bust cycle

A common pattern for individuals with fluctuating physical symptoms is the boom-and-bust cycle. On days when pain or fatigue levels are lower, there is a strong temptation to catch up on missed tasks. This might mean cleaning the whole house, working long hours or running multiple errands in Guildford town centre.

This sudden spike in activity overloads a body that is already compromised. The result is a severe flare-up, or the "bust" phase, requiring days of strict rest to recover. Over time, this unpredictable pattern often leads to muscle deconditioning. The baseline of what the body can handle slowly shrinks, meaning it takes less and less activity to trigger the next crash.

## Pain and nervous system hyper-arousal

Persistent pain changes how the nervous system processes physical sensations. When pain signals are constant, the brain's threat-detection system remains on high alert. The nervous system becomes hyper-vigilant, often interpreting normal, safe movements as dangerous.

This hyper-arousal causes increased muscle tension, shallow breathing and heightened stress, which ironically increases the sensation of pain. Treating the psychological response to pain is a core component of health psychology. By addressing how the brain interprets these signals, it is possible to lower the body's alarm response.

## How cognitive behavioural therapy approaches chronic pain

Therapy for persistent pain management focuses on practical, evidence-informed strategies to stabilise daily routines and reduce secondary distress.

### Activity pacing to find a stable baseline

Rather than letting pain entirely dictate activity levels, CBT introduces time-contingent pacing. This involves stopping an activity based on a predetermined time limit, rather than waiting until the pain becomes unbearable.

If standing causes pain after twenty minutes, a paced approach might involve resting after fifteen minutes. Whether that is managing a short walk around Stoke Park or completing a work task at a desk, the goal is consistency. Pacing helps establish a reliable baseline of activity that avoids triggering the bust phase.

### Graded movement experiments

Fear of movement, known clinically as kinesiophobia, is a highly logical response to chronic pain. Avoidance keeps the body safe from immediate discomfort. In the long term, avoiding movement entirely leads to joint stiffness and further loss of function.

CBT therapists use graded behavioural experiments, which are always carefully tailored to the individual. This involves slowly reintroducing very small, manageable movements to show the nervous system that specific activities do not equal physical damage. Over time, these small experiments help rebuild confidence in the body's capabilities without causing overwhelming flare-ups.

### Cognitive reappraisal and reducing distress

The thoughts accompanying a physical flare-up heavily influence the level of distress experienced. Thinking "I will never be able to walk comfortably again" or "I have ruined all my progress" triggers a strong emotional and physical stress response.

Cognitive reappraisal involves examining these catastrophic thoughts and developing more balanced alternatives. Recognising a flare-up as a temporary setback rather than a permanent failure helps lower stress hormones. This reduction in emotional distress directly reduces physical tension, making the flare-up easier to tolerate.

## Working with a therapist in Surrey

Finding an effective approach to persistent pain management requires understanding your specific physical triggers, thought patterns and daily commitments. A structured approach helps build a more reliable routine, reducing the constant anxiety of waiting for the next physical crash.

We provide individual CBT and counselling (https://cbtandcounsellingsurrey.co.uk/services/individual-therapy) face-to-face across Surrey, as well as through online video sessions (https://cbtandcounsellingsurrey.co.uk/problems/online-counselling) for those who find travelling difficult during health flare-ups.

## Booking your CBT session
